Job Overview:
Reporting to the Program Manager, the Case Manager delivers high-quality, client-centered case management services for individuals and families experiencing homelessness. You will manage a caseload of 15–20 participants, serving as their central point of contact.
Key Responsibilities:
- Conduct initial and quarterly psychosocial assessments.
- Collaborate with participants on individualized service plans and housing stability goals.
- Build trusted, professional relationships with clients.
- Provide outreach, housing navigation, and ongoing support.
- Facilitate referrals to healthcare, mental health, substance use, and community services.
- Support clients in obtaining employment, benefits, and education opportunities.
- Accompany clients to key appointments as needed.
- Use a harm reduction, strength-based, and client-focused approach.
